The above applicant is in good health and has my permission to participate in this program. In case of emergency, I grant permission for my child to be given emergency treatment at a local hospital.*
I hereby release Nathan Davies and First Kicks LLC from all liabilities from any injury or illness incurred at NJ 3V3. *
